Accident Reports

All accidents are to be reported to the College Administration as soon as possible (not to exceed 24 hours) after the accident occurs. The actions taken to prevent a similar accident are to be reported as soon as possible after the hazard or other cause has been corrected.


All accidents occurring on College property (whether owned or rented) shall also be reported using the following form:

A copy of the form is to be sent immediately to Human Resources each time it is completed. Failure to report such an infraction may result in employee disciplinary action, including termination.


Accident with no injuries. Report all accidents to Human Resources and complete an Accident on State Property report per the Missouri Central Accident Reporting requirement.


Student Accidents

For an accident with serious injury:
  • Push the medical panic button or call 911 to request an ambulance.
  • If the injured student is responsive, help him/her contact family members.
  • If the injured student is not responsive,
  • College Counselors, Vice President of Student Affairs, Chief of Staff, or Resident Manager can assist with family notifications.
  • Academic Records can assist with family contact information.
For an accident with minor injury:
  • Push the medical panic button or call 911 to request an ambulance for student to be evaluated.
  • If student needs to be seen by a doctor, but denies ambulance ride, make arrangements to transport the injured student to the Community Health Center/doctor's office/emergency room as needed. (Th injured student should not be transported by another student except as directed in a major crisis or disaster.)
  • Remind the injured student to contact family members as appropriate.

For all student accidents, it is recommended that involved students contact a College Counselor or the Vice President of Student Affairs to complete Incident Reports, using their best recollections.

  • Written statements are given by involved parties and witnesses.
  • Counselors file these reports as proof of students having the opportunity to give facts by making their own statements.
  • Students are given copies of their own statements.
  • Once the students' Counseling Files are retired upon graduation or other exit, all contents including Incident Reports are shredded.


Employee Accidents

For an accident with serious injury:
  • Push the medical panic button or call 911 to request an ambulance.
  • If the injured employee is responsive, help him/her contact family members.
  • Contact Human resources immediately at extension 5224. HR will provide insurance information that the injured employee will need to check in at the emergency room.
  • Begin the process of family notification, if the employee is unable to do so.
  • Notify the injured employee's supervisor, if this has not already been done.
For an accident with minor injury:
  • Push the medical panic button or call 911 to request an ambulance for employee to be evaluated.
  • Make arrangements to escort or transport the injured employee to the Community Health Center/doctor's office/emergency room as needed. (The injured employee should not to be transported by a student except as directed in a major crisis or disaster.)
  • Contact Human Resources immediately at extension 5224. HR will provide insurance information that the injured employee will need to check in at the emergency room.
  • Begin the process of family notification, if the employee is unable to do so.
  • Notify the injured employee's supervisor, if this has not already been done.
